6. Your Signature:

A physical or electronic signature from the copyright owner or an authorized representative.

Filing a Counter-Notice

If you believe that your content was mistakenly removed or wrongly identified as infringing, you may submit a counter-notification. Your counter-notice must include:

Confirmation that you will accept legal service from the party who filed the original DMCA complaint or their representative.
